Human behavioral science project

Hi
I am thinking of conducting survey for my science project on genders . I know I have to take an informed consent from the kids who will participate , what else form do I need. There is no "risk" involved its just a survey with few questions.
Can anyone guide me on this . I am new to science project and all.
