Science Buddies: "Ask an Expert"

Does anyone know what should be generally included on a science fair report title page? I mean obviously the title but like what? our name, school, and teacher conducting the fair?????
PLEASE HELP ME!!!!!!! MY SCIENCE FAIR PROJECT IS DUE MONDAY!
