Consent Forms?

My friend and I E=are doing a project on whether or not fingerprint patterns are inherited, but we can't find any pre-made consent forms. Do we have to ask our teacher for them, or make them ourselves? Thanks. ~ Dusk and Dark

Step 2 indicates to put together a consent form, but as you've noted, no sample is provided.

You might find it helpful to look at the informed consent form from ISEF as an example of the kinds of wording you want to use when you create your form and obtain your signatures:

The link that Amy posted with the ISEF example consent form is down, but you can get that form here:
https://member.societyforscience.org/document.doc?id=33 . Fill in all the sections with an accurate description of what you're trying to study and what you'd like your volunteers to do. Before you make copies, ask your teacher or other adult advisor to review the form and then put their contact information on the form where indicated.
Make copies and ask your volunteers to sign them. Keep the signed copies - you may need to provide them later on when you enter your project in the fair.
