bacterial identification

I have to identify a bacteria based on the following results:
- gram positive
- streptococcus (single, pairs, short chains)
- encapsulated
- colony shape on TSA circular, white, raised/convex
- catalase, oxidase negative
- motile, ornithine positive, indole negative
- facultative anaerobe
- optimum growth at 37
- fermentative, acid, no gas
