I suggest you just look up something like this in your local Yellow Pages, or on-line.

Dry ice is very dangerous to work with, so I don't think that it will be sold freely on the Internet. If you live near a college or university, you might want to go there and ask around.

Yellow Pages are the best reference. I have seen it at the grocery store and sporting goods stores in a locked chest. Take your own ice chest too.
